The Log of Work-Related Injuries and Illnesses (OSHA 300 Log) is used to classify work-related injuries and illnesses and to note the extent and severity of each case.
The Summary, a separate form (OSHA Form 300A), shows the totals for the year in each category.
Employers must keep a Log for each establishment or site. If you have more than one establishment, you must keep a separate OSHA 300 Log and Summary for each physical location that is expected to be in operation for one year or longer.
Recordkeeping in Relation to COVID-19 Vaccinations5/24/2021
With a few exceptions, Oregon OSHA has adopted the federal recordkeeping standard by reference; and keeping in line with Federal OSHA we will not enforce the recordkeeping standard in relation to vaccinations.
Are adverse reactions to the COVID-19 vaccine recordable on the OSHA recordkeeping log?DOL and OSHA, as well as other federal agencies, are working diligently to encourage COVID-19 vaccinations. OSHA does not wish to have any appearance of discouraging workers from receiving COVID-19 vaccination, and also does not wish to disincentivize employers’ vaccination efforts. As a result, OSHA will not enforce 29 CFR 1904’s recording requirements to require any employers to record worker side effects from COVID-19 vaccination through May 2022. We will reevaluate the agency’s position at that time to determine the best course of action moving forward.

The Injury Tracking Application (ITA) is accessible from the ITA launch page, where you can provide the Agency your OSHA Form 300A information. The date by which certain employers are required to submit to OSHA the information from their completed Form 300A is March 2nd of the year after the calendar year covered by the form.
